Self Evaluation
In order to receive an Individual Budget your local authority will provide you with a Self Evaluation Questionnaire. This is a list of questions about you and your life. The questionnaire needs to be completed to determine how much support you need in different areas of your life and how much your budget will need to be in order to cover the cost of this support.
